Humana Medicare Advantage Special Needs Plans
Specialized coverage for your health needs
Humana’s Medicare Advantage special needs plans (SNP) provide medical and prescription drug coverage for people who have specialized health needs. |
Purpose of Special Needs Plans
Medicare special needs plans provide more focused and specialized healthcare for people that require health benefits tailored to their specific needs and conditions. The plans are available to Medicare beneficiaries who have chronic, severe or disabling medical conditions; and those who have both Medicare and Medicaid coverage. Special needs plans are also available to those who live in certain types of institutions (such as nursing homes).
Humana’s Special Needs Plans
Humana offers special needs plans in many areas. These Medicare Advantage plans are designed for people who are eligible for both Medicaid and Medicare, or have been diagnosed with specific chronic conditions such as diabetes or osteoarthritis.
Who Can Enroll in a Special Needs Plan?
Medicare-approved special needs plans are available to anyone who meets the specific eligibility requirements of the plan, and is enrolled in both Medicare Part A and Part B through age or disability. Premiums, copayments, and deductibles may vary based on income. Due to the specific eligibility requirements of special needs plans, they are currently not available for online enrollment.
