You have many choices when it comes to health plans. Why not choose a plan that includes a fitness center membership or activity program? Depending on the Medicare health plan you enroll in, you will receive membership in one of the following fitness programs - at no extra cost!

SilverSneakers^® Fitness Program

When you join one of Humana’s Medicare Advantage or Medicare Supplement plans, you can take advantage of the SilverSneakers Fitness Program. Designed for members at all fitness levels, you can take part in group exercise classes and work with trained advisors who can help you develop a personal fitness plan.

With SilverSneakers, you get:

• Basic fitness center membership at any participating SilverSneakers facility anywhere in the country
• Amenities available with a basic membership, including steam and sauna rooms where available

SilverSneakers^® Steps

If you’re eligible for SilverSneakers and you live farther than 15 miles from a participating SilverSneakers facility, you can enroll in the SilverSneakers Steps program. SilverSneakers Steps is a self-directed physical activity and walking program. It provides the equipment, tools, and motivation for you to measure, track, and increase your activities and achieve a healthier lifestyle.

With SilverSneakers Steps, you get:

• A pedometer for counting daily steps
• An activity planner and tracking log
• Incentive prizes
• Healthy Steps quarterly newsletter that includes passes for free visits to fitness centers and invitations to community events that promote a healthy lifestyle

The SilverSneakers Fitness Program and Steps Program are available with most of Humana’s Medicare Advantage plans. However, they are not available with all plans in Arizona, California, Delaware, Maryland, New Jersey, and Pennsylvania, or the HMO plan in Dallas, Texas.

Silver&Fit^TM*

Humana Medicare Advantage and Medicare Supplement plan members in Arizona and Pennsylvania can join Silver&Fit, a program that incorporates exercise and health education to empower you to become physically fit.

With Silver&Fit, you get:

• Basic membership at a local participating fitness club, which includes access to equipment such as weights and cardiovascular and resistance training equipment
  
  
• Group fitness classes designed specifically for seniors and focus on aerobic, flexibility and strength-training exercises
  
  
• Healthy aging classes with educational materials to help you make better health decisions
  
  
• Community social activities

Silver&Fit@Home^TM

If you're eligible for Silver&Fit and you live farther than 15 miles from a participating Silver&Fit location, you can enroll in Silver&Fit@Home. Silver&Fit@Home is a home-based fitness and health education program that offers a choice of either an exercise program or a walking program.

Silver&Fit and Silver&Fit@Home are available with Humana's Medicare Advantage and Medicare Supplement plans in Arizona and Pennsylvania.

Humana Gold Choice and Medicare Supplement plan members in California, Delaware, Maryland, New Jersey, and the Humana Gold Plus plan in Dallas, Texas, have access to fitness and wellness resources through Forever Fit – a program that can help you take charge of your health.

With Forever Fit, you get:

• Membership at a participating fitness center, including Curves for Women, 24 Hour Fitness, Gold’s Gym, Club 50 Fitness and more
  
  
• All amenities and programs associated with a standard membership
  
  
• National reciprocity at nearly 8,000 participating fitness centers

The Forever Fit program is available with Humana Gold Choice and Medicare Supplement plans in California, Delaware, Maryland, New Jersey, and the Humana Gold Plus plan in Dallas, Texas.
