Manage the daily impact of chronic illnesses

Members with complex health conditions need personal attention. Medicare Advantage members have access to the following special clinical programs, which are designed to meet each member’s unique needs.
We identify program participants in many ways, including referrals from other health resources programs and services. Humana clinical programs help to ensure the best quality of life and health outcomes possible:

Transplant management services
When a member faces the prospect of receiving a solid organ or bone marrow transplant, it is a time filled with questions. Those who are touched by organ transplantation can rest easier knowing Humana’s Transplant Management department focuses on the patient's transplants needs.
Transplant Management is a dedicated team of registered nurses who are the single point of contact for transplant related services. These nurses guide members to our National Transplant Network, maximize benefits, coordinate transplant related care, and authorize transplant related services from evaluation until one year post transplant.
Complex case management
Humana’s services give specialized focus to members with complex medical situations and hospitalizations.
We work with members to close gaps in care and to help overcome possible hurdles or barriers in the path to a successful recovery.
Disease management
Chronic diseases require a coordinated and multi-dimensional approach to care. Our goal is to help members by providing education and information so they can improve how they work with their doctor to manage their conditions and avoid complications.
That’s why Humana has developed an unparalleled number of disease management programs that address more than 20 conditions, including:
- Congestive Heart Failure
- Chronic Obstructive Pulmonary Disease (COPD)
- Diabetes
- Asthma
- Chronic kidney disease and end-stage renal disease
- Cancer
- Rare diseases (including cystic fibrosis, Parkinson's disease, and multiple sclerosis)
