Humana Inc. is a licensee of the TRUSTe Privacy Program. This Internet privacy statement applies only to information on http://www.humana-medicare.com, http://www.humanagold.com and http://medicareplans.humana.com.

TRUSTe is an independent, non-profit organization whose mission is to build users' trust and confidence in the Internet by promoting the use of fair information practices. Because this Website wants to demonstrate its commitment to your privacy, it has agreed to disclose its information practices and have its privacy practices reviewed for compliance by TRUSTe. Personal Information

At Humana Inc. (and its wholly owned subsidiaries), we are very aware of the privacy concerns of our customers and others who visit the Humana Website. In fact, we share your concerns and will not solicit personal information from you as you tour our Website, though you may provide that information voluntarily. If you do elect to provide personal information to us, in no case will we sell, license or transmit that information outside of Humana Inc. for any reason, unless you authorize us to do so.

There are instances in which Humana Inc. might internally use the personal information you voluntarily submit. These instances include identifying and registering you for the Self-Service Center portion of our website. Personally Identifiable Information is described as data that is unique to an individual, such as a name, address, telephone number, social security number and date of birth (DOB). At times, we may request personal information from you through our Websites in order to deliver requested materials to you, respond to your questions, or deliver a product or service. Members, physicians, employers, business partners and agents have the ability to register in a secured area of our website in order for us to deliver such products or services. Additional information collected by these Websites may include organization names, addresses, group numbers and tax identification numbers. The purpose of the registration is to validate members, providers, employers, business partners and agents requesting access to our secured area. The registration also enables us to tailor the information sent to you. Contact forms are used only to deliver requested materials and respond to your questions.

Prospective members have the ability to register within the secured online application portion of our Website. Information collected within this portion of the Website will be used for plan validation and enrollment purposes and may include the prospect member's name, address, phone number, DOB, social security number, Medicare id number, Medicaid id number, current insurance coverage, current health conditions and other pertinent information. Additionally Humana may obtain information from third party sources such as a credit bureau or fraud protection service agency to verify the user's identity.

By providing an e-mail address, you are indicating that publications can be sent to your e-mail address. You are not required at any time to submit your e-mail address for the purpose of receiving recurring informational/promotional e-mails from Humana Inc. and/or third parties. Health plan members may opt out of receiving publications via e-mail by contacting the Customer Service toll-free number that appears on your member identification card or indicating your preference on our Website. Humana Medicare applicants may opt out of receiving publications via e-mail by contacting the Customer Service group at 1-800-833-6578.

Health plan members, at the time of login to the Self Service area, you have the option to change your password and e-mail address. In case you forget or misplace your password, we will use your Secret Prompt and Secret Response to identify you in order to provide this information to you. Your Personally Identifiable Information can be changed by contacting the Customer Service toll-free number that appears on your member identification card.

We collect information about you and your use of our Website through the use of session and persistent cookies. Session cookies are small text files that are stored within your computer's temporary memory. These temporary files are used to facilitate your navigation throughout the website. Additionally, session cookies are used to grant you access to the secured portions of the Website. Access to the secured portions of the Website will not be granted if the session cookie is rejected.

Persistent cookies are small computer files that are transferred to your computer's hard drive. These stored files are used in the generation of data that is fed to a contracted third party for Website usage analysis. Reports created using this information will provide Humana with a better understanding of the use of this Website. Your encrypted (encoded) information is protected using "Secure Socket Layers (SSL)" as it passes between your browser and the Humana Inc. servers. E-mail

Should you send us your questions, comments and job inquiry via e-mail to any mail box provided on this Website; we will share your correspondence with our Customer Service representatives and those employees or agents most capable of addressing your questions and concerns. We will retain your communication until we have done our very best to provide you with a complete and satisfactory response.
